What do you need to bring?
- ID (ID card, passport or residence permit)
- A valid insurance card
- Physician's note
Registration at the reception
You will first be given an access card to the Clinical and Research Centre for Inflammatory Bowel Disease (8th floor) at the main reception. At the Inflammatory Bowel Disease Clinical and Research Centre reception you will be asked for proof of identity and an insurance card, which will be used to allocate and give you your identification PIN and a Patient Consent Record for Disclosure of Health Information (GDPR).
What is a PIN?
PIN (four-digit security password), which the patient receives at the reception when signing the GDPR form. This is the caller identification when enquiring about the results of the test and then the password to open the sensitive data file in email communication.
Which patients do we accept for care?
- We only accept patients with a diagnosis of chronic intestinal inflammation - Crohn's disease and Ulcerative colitis.
- An appointment for endoscopic examinations can only be made on the basis of a physician's request. We do not perform examinations at the patient's request; a doctor's indication is always necessary.
How do I apply for admission to outpatient care?
- Send your medical records - last outpatient report, MR/CT result, report from the last endoscopic examination or sonographic examination - to recepce@iscare.cz.
- Please put "request for admission to care" in the subject line. Do not forget to include a contact telephone number.
